Diabetes Self-Management Program

Congratulations to the first graduating class of the Refuah CHC’s Diabetes Self-Management Program! The program, offered in collaboration with Konbit Neg Lakay Haitian Community Health Center, is a 6-week evidence-based course led by peers who have undergone 5 full days of training. Participants celebrated their completion of the first series, which was held in French-Creole, with a memorable graduation ceremony on April 25, 2017 at Konbit Leg Lakay Haitian Community Center.  Given the opportunity to reflect on the experience, one graduate shared, “I never in my life had my diabetes under control.  Thank you for this program.  I feel like a new person”.

The second 6-week series, being held in Spanish,  has already begun. All classes are held at Konbit Neg Lakay and are open to the public.  Participation is dependent on class size and availability.
