Diabetes Self-Management Program

The first graduating class of the Refuah CHC’s Diabetes Self-Management Program, offered in French-Creole in collaboration with Konbit Neg Lakay Haitian Community Health Center. The program is a 6-week evidence-based course led by peers who have undergone 5 full days of training. Given the opportunity to reflect on the experience, one graduate shared, “I never in my life had my diabetes under control.  Thank you for this program.  I feel like a new person”. Click here to see a 3 minute clip about the inception of this project.
